1.Acceptance of Terms

2. Services Provided

We provide outpatient psychiatric and mental health services, including but not limited to:

  • Medication management

  • Telehealth services

  • Spravato (esketamine) treatment

  • ADHD assessments (including cash-based services)

  • Medical marijuana consultations

Services may vary by state and provider availability.

3. No Emergency Services 🚨

This practice does not provide emergency care.

If you are experiencing an emergency:

  • Call 911, or

  • Call/text 988 Suicide & Crisis Lifeline

Do not rely on this website, email, or text messaging for urgent concerns.

4. Appointments, Cancellations & No-Show Policy

  • A minimum of 48 hours’ notice is required for cancellations

  • Same-day cancellations and missed appointments are considered no-shows

  • No-show fees may apply

We reserve the right to limit or discontinue services for repeated missed appointments.

5. Payment Terms

  • Payment is due at or before the time of service unless otherwise arranged

  • We accept insurance and self-pay depending on the service

  • You are responsible for verifying your insurance benefits, including:

    • Deductibles

    • Copays

    • Coverage limitations

Failure to pay may result in suspended services.

6. Cash-Based Services (Strict Policy)

For services including ADHD assessments, MMC, and other self-pay services:

  • All payments are non-refundable once the service process has begun

  • This includes:

    • Completed visits

    • Issued testing links

    • Administrative processing

7. ADHD Assessment Disclaimer

ADHD testing services:

  • Are not diagnostic on their own

  • Do not guarantee medication prescriptions

  • Are intended to provide objective data to support clinical evaluation

Results should be reviewed with a qualified provider for diagnosis and treatment planning.

8. Spravato (Esketamine) Treatment Terms

For patients seeking Spravato treatment:

  • A referral from a treating provider is required

  • Medical records must be submitted for review

  • Eligibility is determined after clinical evaluation and benefits verification

Important Requirements:

  • Patients must have transportation arranged (no driving after treatment)

  • Failure to comply with safety requirements may result in cancellation of treatment

Coverage and approval are not guaranteed.

9. Communication Policy

By providing your contact information, you consent to receive:

  • Text messages

  • Emails

  • Phone calls

Important Boundaries:

  • Messaging is for non-urgent matters only

  • Excessive or inappropriate messaging may result in communication restrictions

  • Emergencies must be directed to appropriate emergency services

10. Telehealth Terms

By participating in telehealth services, you agree:

  • To be in a private, secure environment

  • To use a reliable internet connection

We are not responsible for:

  • Technical issues on your end

  • Privacy breaches due to your environment or device

11. Prescription Policy

  • Prescriptions are issued based on clinical judgment only

  • Controlled substances are prescribed at the provider’s discretion

  • Early refills or replacements are not guaranteed

12. Limitation of Liability

We are not liable for:

  • Website interruptions or errors

  • Third-party services (payment processors, pharmacies, platforms)

  • Outcomes of treatment decisions

All services are provided “as is” within the scope of professional standards.

13. Intellectual Property
